Abstract Aims Acute appendicitis is a common emergency surgical presentation accounting for approximately 30,000 operations annually in Australia. There need clinical grading tool that can quickly and reliably assess the severity of at time surgery predict difficulty operation. Methods Over 12 months, 111 questionnaires relating to laparoscopic appendicectomy operation, anatomical pathological features, taken, senior assistance were completed by surgeons trainees varying seniority. Construction scale operation was method summated ratings. The final generated utilising further item analysis, principal components internal consistency reliability analysis concurrent validity analysis. Results A 8 features formed. These include presence acute adhesions, gross pathology appendix, quality mesoappendix, appendix base retroperitoneum, visibility artery, adherence adjacent structures. has high reliability, both individual items been validated comparison with surgeons’ perceived difficulty, length call assistance.
